Our unique services are designed to make it easier for patients to receive care in their own homes. We provide:
- Coordination with physicians, nurses, hospital discharge planners and insurance companies.
- Recommendations for appropriate state-of-the-art therapies, equipment, and care plans.
- Monitoring and evaluation of clinical responses to treatment.
- Frequent updates to the physician on patient progress.
- Specialized training and education for patients and caregivers.
- Delivery of all drugs, equipment, and supplies, to the patient’s home.

Patients receiving home infusion therapy through PHI will receive up to two nursing visits per week to asses and provide appropriate nursing care to the catheter site, obtain lab work ordered by the physician, and perform a complete, systems based physical assessment. If a patient and his or her family cannot administer the IV therapy, PHI’s nursing staff will work with the patient and his or her family to enhance their skills and build their comfort to a level that will enable them to perform the infusion independently.
- After receiving a patient referral our nurse makes an initial assessment of each patient
- Therapy begins with extensive Patient Education and training on equipment, aseptic technique and safe administration
- Catheters are placed by our certified nurses when indicated
- Nursing visits are made until the patient is comfortable with all aspects of home care. Our nurse monitors and supports each patient throughout the Care Plan to ensure a successful outcome, and afford each patient the highest quality of life during therapy
- The nurse works with the pharmacist and physician to coordinate all aspects of care and make adjustments as needed

All co-pays and deductibles will be discussed with the patient prior to the initiation of therapy.
A registered nurse will be the primary contact person with both the physician’s office and the patient throughout the case. Any case management services or prior authorizations required by insurers will be obtained by PHI.
